Printable car accident checklists and forms

Paper still works when a phone is cracked, dead or locked. These sheets print in black and white, fit Letter or A4, and leave out ads and site menus.

What to keep off paper

A card in the glovebox can be read by anyone who opens the car. Leave off your full Social Security number, bank or card numbers, and detailed medical history. A short allergy note or a medical condition that first responders should know about is usually enough. Your insurer's claims phone and your policy number are the details you'll need most.

Printing tips

  • Use your browser's print dialog. Set scale to 100% (or “Actual size”) so the glovebox card prints at true card size.
  • Choose “Save as PDF” in the same dialog if you want a file to keep on your phone or send to family.
  • Laminating the card, or putting it in a plastic sleeve, keeps it readable after years in a hot car.

For what each step means and why, read what to do after a car accident. All letters you can fill in are on the templates page.
